5. GUEST SPEAKERDr MS Salduker (Psychiatrist)The chairman introduced Dr Salduker the resident psychiatrist at Entabeni hospital. The notes hereunder are from the presentation and from questions that were asked by those present. The doctor reminded us of the difference between a psychologist & a psychiatrist, one dealing with the mind (projection of brain – abstract) & the other (psychiatry) dealing with the function of the real thing – the brain. It is common that patients are referred for assessment either prior or post bypass operations. A bypass patient has 2 levels of change in the brain – anxiety, having to deal with this massive operation & the other change being the possibility of depression.Depression – is a clinical observation, dealing with the cognitive memory, temperament change, change in energy level, agitation.Anxiety – deals with worry, fear or phobia – subtle in brain, which is the result of constant worrying & wants to avoid or prevent the danger (of the operation). Stress is often referred to as a disease, but is an associated anxiety, where the brain is telling the body.Anxiety is the “fright, flight syndrome”, where the response can be seen through the entire body – heart starts beating faster, breathing then becomes faster, muscles can become tense & all this can be done in the fraction of a second where the brain is sending signals to the body. All these dramatic & quick changes in the body, draws blood from all the non-essential organs & transfers the blood to the muscles. So if a person has constant anxiety then, the heart pressure is constant & therefore cannot cope.Anxiety also causes such things as irritable bowel syndrome & in some cases GPs treat the patient by prescribing medicines & miss the problem of anxiety. Anxiousness can therefore cause high blood pressure & as the brain controls the body, a panic attack can cause asthma, which at the end of the day is brought about by anxiety. A person becomes anxious when they hear or feel that there is something not right with their heart & can develop a phobia, which is some times the reason that these “heart murmurs, pains, etc” and which do not reveal anything on the ECG, as an anxiety attach does not have to have a reason to attack – it is a rapid automatic response, which increases the heart beat and/or causes sweating, etc.. So a panic attack – can result in a sharp chest painAnxious people develop heart problems – 1 in 3 people have some form of anxiety from mild to paranoia.Sense of doom generates anxiety & a panic attack feels like a heart attack as a result of the adrenal levels increasing & the serotonin levels decreasing. In the USA half of the cardiac patients are being treated for anxiety.Another common problem is people who suffer from constant tension headaches, which affects the spine – we need to learn to relax, as the first organ to suffer is the brain, which can develop brain disorders. We only use +- 18 to 21% of our brain & we start to lose our memory from (Ed - can’t remember) – sorry from the age of 38 – brain cells do not regenerate. Exercise keeps the brain healthy. In a lot of the cases these panic attacks can cost the patient R30k, as the diagnosis is not cardiac. It is therefore important to look after our mental health & not just our diet, exercise & stress, etcThank you Dr Salduker, for a very interesting & enjoyable talk.
NEXT MEETINGo The next meeting would be on Wednesday the 2nd of June 2010 at the Conference suite at the Entabeni Hospital at 17:00 for 17:30.

5. RAFFEL DRAWThe raffle, was won by Danny Naidoo. Shane apologized that he had stated in the minutes that Athol Willet had won the raffle at the last meeting. This was an error, as George Barns won it.
GUEST SPEAKERHeart of the Matter by Dr Cassim HansaThe chairman introduced Dr Cassim Hansa, the resident cardiologist at Entabeni hospital. The notes hereunder are from the presentation and from questions that were asked by those present.A presentation was made by Dr Hansa and he started by saying that there is a connection between the physical & metaphysical of the human being of the heart and brain – “feelings from the heart”, “broken heart”, “wearing ones heart on their sleeve”, etc. The human heart develops in the fetus at 12 weeks & at this stage, no brain has been formed. The heart pumps +- 300l of blood an hour, +-100 000 beats per day, through a blood vessel system that is +-100 000 km long. The heart is similar to the motor car engine with valves & an electrical system.Dr Hansa went on to explain how the heart pumps, by means of his presentation.A cardiologist is like a plumber & deals with the “corrosion” of the human “pipes” (atherosclerosis) – cardiologists do not cure heart disease but alleviate the problems. Bypasses can increase & improve your lifespan, whereas stents decrease the symptoms but do not increase lifespan. Stents do stop working, for what they are intended to do, over time, but cannot be removed or replaced – can lead to a bypass in some cases or insertion of more stents. When cardiologist insert stents the arteries can be blown up to 18 – 21 bars – a considerable pressure, bearing in mind a car tyre takes 2 kph. Most stent patients are placed on Plavex (Anticoagulants medication) for at least 1 year & the bruising which occurs is a good sign that the medication is working. Plavex should only be stopped if in an emergency. A Beta Blocker like Carloc slows heart rate, as scar tissue (heart muscle dies after a myocardial infarction (heart attack) can affect heart rate. It is advisable to see your cardiologist at least once a year. One of the biggest problems with bypass patients is not completing a rehabilitation program and living in fear. Most medical aids will contribute to the costs of rehabilitation for six months, after a bypass. Fat is a major problem for the heart, as more blood has to be pumped through more capillaries, causing the heart to work more, fat collects around vessels, the heart & other organs, causing pressure on these organs. As Dr Hansa had patients to see, the members appreciated & enjoyed the time he had taken to speak to us. Thank you Dr Hansa, for the enjoyable talk.
NEXT MEETINGThe next meeting would be on Wednesday the 3rd of February 2010 at the Conference suite at the Entabeni Hospital at 17:00 for 17:30. The next gathering of members would therefore be at the DCC for the annual Christmas & End of Year function at Durban Country Club on the 29th of November 2009. The meetings for 2010 will be as follows :o 3 February 2010o 7 April 2010o 2 June 2010o 4 August 2010o 6 October 2010o 28 November 2010 (Annual luncheon)Kindly diarize the above dates. Please support your club.

3. TESTIMONIES FROM NEW VISITORSAlan Pather – Alan had been a road runner moist of his life, fit, had completed his Comrades and 6 years ago while on a run uphill & started having a bit of difficulty – started battling to even climb stairs. He underwent a stress test & an “angio” revealed that he had damaged arteries & was placed on medication to repair the damage – he gave up running. A year ago, while chairman of his running club, he woke up with “wind” – after 2 litres of Soda water, he collapsed at the doctors rooms. He is recovering well after his bypass & has started running again.· Colin Hall – Colin started having a difficult time firstly with what he thought was a “mossie” bite, which turned out to be a spider bite – within 3 hours he was placed on a drip & antibiotics. He then had a reaction to the antibiotics, suffered heart attack and received a bypass in March and 2 days later had a stroke. This man has made a remarkable recovery, after all this, in such a short space of time & the club wishes him all the very best in his recovery.· Sheldene Stuart –Sheldene had no history of cholesterol or any cardiac disease & was diagnosed with 2 “blockages” – the stents inserted failed & after receiving a bypass, developed pneumonia. Sheldene will be providing a service to the Umhlanga hospital – welcome Sheldene & many thanks for your willingness to assist those, who may need some encouragement & somebody to listen to either before their bypass or after, at Umhlanga.· Lynn Alborough – Lynn had his bypass 5 weeks ago & is recuperating well.We welcome & wish them good health.

The chairman requested for a vote on whether, as mentioned that if the club was prepared, the club could pay for the tablets, which would be in the region of R12k, but as there were an insufficient number of members, who by show of hands, would be prepared & wanted to take the tablets, the chairman made a decision, based on the show of hands & after a lengthy debate, that no tablets would be purchased by the club.
